Minority Health Initiative: Path to Good Health & Wellness

It starts with YOU!

You can gain knowledge and lower your personal health risk factors by attending education sessions on nutrition and physical activity. Learn to choose nutritious foods and beverages and how to cook and purchase alternative healthful food on a budget. Referrals for medical homes, routine and specialty dental care will be made with community health workers, but you must take that first step to reach out and make contact! All services are income-based.
